  • Shannon Harris - Owner/Therapist/Facility Director

  • Discipline: Speech Language Pathologist, MS, CCC-SLP
  • Education: M.S., Speech-Language Pathology, West Virginia University 2003, B.S., Speech Language Pathology and Audiology, Purdue University, 2000. Shannon has twelve years of experience in Speech Language Pathology services from pediatrics to geriatrics. Shannon specializes in dysphagia, and is certified in neuromuscular facilitation for dysphagia pediatrics and geriatrics. CFY supervisor for new graduates in Extended Care Facilities, She is married with one son.
  • Dr. Sivaprasad Parnam, DPT - Owner/Therapist

  • Discipline/Certifications: DPT, Cert.SMT, Cert.DN, CSRS, CCI,.
  • Education: He received his undergraduate degree in health sciences from RajivGandhiUniversity in India and his Doctor of Physical Therapy from Dominican College of Blauvelt in New York.


  • Dr. Siva has many certifications that enable him to offer a higher level of expertise to his patients, including certifications in vestibular rehabilitation therapy, spinal manipulative therapy and Dry Needeling. Dr. Siva is also a Certified Stroke Rehabilitation Specialist and a Credentialed Clinical Instructor. He and his wife, Sujatha, have a daughter and twin sons. Dr. Siva is a Rotarian and enjoys participating in community events. .
  • Alfredo Enriquez - Owner/ Occupational Therapist

  • Discipline: Occupational Therapist, OTR/L
  • Education: B.S., Occupational Therapy, Perpetual Help College, Philippines 1997. Alfredo was a top placer when he took the Philippines OT Licensure examination. He was an instructor specializing in splinting application at the Philippines College of Health Science, Fatima University, Perpetual Help College of Manila, and Saint John and Paul College. Alfredo has fifteen years of experience in Occupational Therapy providing services from pediatrics to geriatrics. From 2002 to 2006, he worked in New York schools and early intervention programs. In 2006, he moved to Indiana and practiced in geriatric settings. Alfredo is Sensory Integration and Praxis Test (SIPT) certified, a supervisor to COTA implementing plan of cares, he is trained in mobility assistive devices, and a passionate advocate for providing services to patients with dignity. He is married with two daughters.

  • Education: Masters in Physical Therapy, Nova Southeastern University 2000, B.A. Movement and Sport Science/Exercise Fitness., Purdue University 1997. John has seven years of experience in Physical Therapy providing services from pediatrics to geriatrics. John was a Clinical Mentor covering the Midwest Region for a Long Term Care Company in-servicing both PT's and OT's for Urinary and Bowel Incontinence. He has continuing education in wound care, geriatric mobility, and Urinary/Bowel Incontinence for all age ranges. He is married with a son and daughter.

  • Education: M.S., Speech Language Pathology., Saint Xavier University 2007., B.S., Speech Language Pathology and Audiology 2005. Jessica has five years of experience working with pediatric population, from one year of age to 18 years of age. She is the Speech Language Pathologist for the Rensselaer Central School System. Jessica is Autism PEC communication trained, and is a Clinical Fellowship Year Supervisor for new graduates. She is married with two daughters.
  • Jasmin Enriquez - Occupational Therapist

    Education: Graduated from Fatima University in 1998 with B.S. in Occupational Therapy. Jasmin worked as a clinical supervisor at Pasig City General Hospital Rehabilitation Department (1998- 2001), Worked with geriatric therapy setting at Heritage Healthcare in West Lafayette, IN (2006-Present). She is trained in mobility assistive devices. She is also certified in Clinical instructorship for Occupational Therapy. Jasmin is married with 2 wonderful daughters.

  • Education: DPT., Alabama State University, Physical Therapy, Indiana University Bloomington 2008., B.S. Janell practices in outpatient clinics including sports medicine, and Extended Care Facilities in Indiana. She loves to travel.
  • Sharlyn Tolentino - Physical Therapist

    Education: Graduated from Fatima University in 1997 with B.S. in Physical Therapy. A versatile physical therapist with 12 years of experience in providing inpatient and outpatient rehabilitative services for adult and geriatric clients. Sharlyn is a certified wound care specialist. Her specialty is in wound care, total joint replacement and orthopedic physical therapy. She is married and gifted with one son.
  • Julia Farrell - Community Management Specialist

  • Julia Farrell has been involved in the medical arena for 8 years. Previously she worked as a procedure specific representative involving Smith & Nephew Endoscopic medical equipment. Past experiences also include working as a Resource Representative for other product lines including rehabilitation equipment and wound closure technology. She understands the importance of educating the community about the benefits of rehabilitation services. Julia is a patient advocate and has a passion for encouraging health and healing. She loves to promote the physical, emotional, and cognitive benefits of physical exercise. Julia is a resident of West Lafayette, where she lives with her husband and two daughters.   • Priscilla Miller - Business Office Manager

  • Priscilla (Pixie), is the Business Office Manager, and has twenty-one years of billing experience. She has been a long-time advocate for policy holders, whether they were Extended Care resident's or outpatients. Her savvy business ethics warrants appreciation, evidence by insurance coverage verification, with reimbursements in a timely manner. Her experience includes: applying for provider networks, filing electronic claims for Medicare, and private insurances, calculations of Medicaid spend down allowance, payroll, quarterly tax preparation, account holdings, invoice documentation, and appealing denied claims.
